We are looking for a motivated forward thinking practice pharmacist with primary care experience to join the multi-disciplinary team at West Quay Medical Centre. The practice team consists of 7 GP partners, a salaried GP, 1 Registrar, Advanced Nurse Practitioner, Nurse Practitioner, 3 Practice Nurses, 2 HCA’s a Cluster Pharmacist and a very supportive admin and reception team.
Working as part of an established and supportive clinical team you will be undertaking a range of prescribing duties with excellent opportunities for continued professional development, mentorship and support.
The role will be largely patient facing, and you will be expected to undertake clinical medication reviews, answer medication queries and manage repeat prescription authorisations and re-authorisations. You will also provide prescribing advice and support to the practice clinical team which will include undertaking audits, developing protocols, providing feedback to prescribers and monitoring improvements.
As part of your role you will also help patients to manage their long term chronic conditions as well as supporting support the shared care and anticoagulation medication process. As such it is essential for you to have an Independent Prescribing qualification and previous primary care experience.
